Impact: Although the uptake of ridesharing was low and did not decrease missed primary care appointments for this particular clinical trial, future studies trying to reduce missed appointments can explore alternative delivery models or target populations with stronger transportation needs.

Goal: The goal of Birth and Beyond California is to provide effective, evidence-based breastfeeding support to all mothers delivering babies in hospitals in California in order to increase the percentage of mothers who are able to breastfeed exclusively starting at birth.

Goal: The goal of CareSouth Carolina and Northeastern Rural Health Network is to improve the behavioral health wellness of patients and residents.

Impact: The partnership between CareSouth Carolina and Northeastern Rural Health Network has reduced the stigma regarding mental health issues, improved care coordination, and increased the likelihood that patients receive the physical and mental health support they need in order to be healthy.

Goal: The goal of the Extreme Recruitment program is to use MFCAA's expertise in facilitating adoption and kinship placement to find permanent, successful forever homes for foster children who would otherwise remain in foster care or residential treatment until adulthood.

Impact: In its pilot year, the program successfully placed 22 children in permanent homes.

Goal: To assess the impact of affordable housing on health care outcomes in a low-income population who have experienced housing instability.

Impact: Costs to health care systems were lower after people moved into affordable housing. Primary care visits went up after move-in; emergency department visits went down. Residents reported that access to care and quality of care improved after moving into housing.

Impact: The Community Preventive Services Task Force (CPSTF) recommends depression care management in primary care clinics for older adults with major depression or chronic low levels of depression (dysthymia) on the basis of sufficient evidence of effectiveness in improving short-term depression outcomes.
